I would have expected safety glass to be unable to shatter into daggers and shards. I thought the entire point of safety glass is that it will only break into small blunt pieces.

i havent seen a broken washing machine door , ive been doing repairs for 25 years now .. but theres no smoke with fire ..

i think unless you happen to be at the appliance when it breaks its very hard to say whats causing this..

or maybe we need to data base of what appliances is breaking the most ..
